Welcome to Shifa Wellness LLC. By accessing or using our services—including Adult Rehabilitative Mental Health Services (ARMHS), Non-Emergency Medical Transportation (NEMT), and MNsure Navigation—you agree to comply with the following Terms and Conditions. If you do not agree with these terms, please do not use our services.

  1. Services Provided

Shifa Wellness LLC offers community-based and person-centered services in Minnesota:

  • ARMHS: Mental health rehabilitation services for adults with mental health diagnoses to restore independent living and community integration skills.
  • NEMT: Transportation services for eligible clients to and from medical, mental health, and social service appointments.
  • MNsure Navigation: Free, certified assistance with enrolling in public and private health plans via the MNsure health insurance marketplace.
  1. Eligibility Requirements

To access our services, you must:

  • Be a Minnesota resident;
  • Meet specific program eligibility criteria (e.g., a qualifying mental health diagnosis for ARMHS);
  • Receive authorization or referral where required by the State of Minnesota or your Managed Care Organization.
  1. Use of Services

Clients agree to:

  • Provide accurate, current, and complete information;
  • Attend scheduled appointments and be present during service delivery;
  • Notify Shifa Wellness LLC of changes or cancellations at least 24 hours in advance when applicable.
  • Improper use of services, including false information, repeated no-shows, or misconduct, may lead to service discontinuation.
  1. Billing and Payment

Shifa Wellness LLC bills approved payers, including Medicaid and other Minnesota Health Care Programs. Clients will not be charged out-of-pocket unless a service is not covered, in which case all costs will be disclosed prior to delivery.

  1. Confidentiality and Records

We protect all client data in accordance with HIPAA and Minnesota state privacy laws. Personal and health information will only be shared with authorized individuals or agencies for purposes of care, coordination, billing, or legal compliance.

  1. Limitation of Liability

Shifa Wellness LLC is not responsible for:

  • Client-provided misinformation;
  • Delays or missed appointments due to external factors;
  • Personal items lost or damaged during transportation services.
  1. Modifications to Terms

We may update these Terms periodically. The latest version will always be available on our website and upon request.
